Matthew 25 AIDS Services is a non-profit healthcare clinic that specializes in the treatment of HIV/AIDS. We believe that nobody should face these diseases alone. Matthew 25 AIDS Services exists for the sole purposes of providing high quality services for those affected by HIV/AIDS.
The successful candidate will share Matthew 25 AIDS Services, Inc.’s commitment to its mission, to support, educate, and treat those infected and affected by HIV/AIDS.
The Staff RN will carry out responsibilities which insure high quality, well-coordinated patient care for our population of patient’s that do not have an undetectable viral load. They will work closely with the clinic team to insure activities related to patient care, and support staffing is optimized for efficient, compliant provision of high quality patient care. The Staff RN will also take responsibility as the OSHA coordinator for the organization and will participate in the operation of the Employee Wellness Program, M25fit.
As Medical Case Manager:
Will work directly with our population of patient’s that do not have an undetectable viral load in improving medication and appointment adherence.
Maintains patient care schedule for those patients in ECW (E-Clinical Works) and keep team members apprised of schedule fluctuations related to patient demand or staffing levels.
Collaborates with clinic team in program goal setting, progress evaluations, and ongoing problem solving.
Participates in Clinical Care Conference, staff meetings and trainings. Will update the multi-disciplinary team during Clinical Care Conference with regards to patient eligibility concerns or issues as provided by the Eligibility Specialist.
Assists in completing written and phone notification to patients of follow up due or in obtaining documentation from other providers of referral or follow up outcomes.
Is responsible for completing prior authorizations for patients.
Develops patient-focused case management plans.
Documents patient’s case management plans and on-going activities in ECW.
Will provide case management services for Part C/D only clients, such as completing outgoing referrals and sending medication vouchers to pharmacies.
Identifies patient’s insurance coverage or other sources of payment for services.
Identifies and addresses client risk factors and/or obstacles to care in an on-going period.
Identifies client needs, current services, and available resources, and then connects the client to services and resources to meet established goals.
Communicates the care preferences of patients serving as their advocate, and verifies that interventions meet the patient’s needs and goals of treatment.
Screens clients and/or population for healthcare needs.
Educates the client/family/caregiver about the case management process and evaluates their understanding of the process.
As OSHA Coordinator:
Will plan and lead OSHA committee meetings and create documents such as minutes and agendas.
Administrator of SteriCycle site along with the Compliance Officer.
Adds new staff to SteriCycle for OSHA training.
Coordinates OSHA annual training and maintain documentations.
Obtains documentation or administers new employee vaccinations, and annual vaccines such as flu shot, and TB skin test.
Reviews and maintains incidents, incidents reports with OSHA committee.
Files medical documents for employees in the HR office.
Logs and scans employee trainings into Bamboo and files in HR files in HR office.
Maintains all safety data sheets.
Checks all fire extinguishers monthly and verifies offsite location checks and sign off.
Manages biohazard procedures, schedules biohazard pick up and all hazardous waste disposal.
Maintains and checks sharps containers.
Maintains sharp injury log.
Orders necessary PPE, and provides PPE training for all new staff.
Orders clinic/medical supplies for all sites.
Maintains eyewash station and documentation of flushing tests weekly.
Plans and executes monthly drills.
Understand and abides by all OSHA record retention requirements.
Develops and maintains emergency preparedness plans and first aid policies.
Approves use and labels small office appliances.
Enforces safety rules and occupational health standards.
Reviews and maintains all Safety Policies.
Will Attend Emergency Preparedness meetings in Owensboro.
Will execute weather radio inspections, and maintain emergency preparedness supplies.
Will be required to participate on the Employee Wellness Committee and assist with the Program, M25fit.
The RN will have a ADN or Bachelor’s degree and have license to practice in Kentucky and Indiana.
Two years of experience in a clinical setting
Candidates with relevant work experience or advanced education in public health, home health or outpatient ambulatory care setting or another relevant field will be considered with preference.
The Staff RN must have highly developed verbal and written communication skills and the ability to work effectively with administrative, clinical and support staff to accomplish the goals of the program.
Knowledge in HIV or HCV a plus.
